Occupational Health and Safety Administration alerts- A workplace accident at Horner Electric left a worker with fatal head injuries.

Indianapolis, IN—A workplace accident left one employee with a potential traumatic brain injury (TBI), which later proved fatal after he was found laying on the ground at Horner Electric. David Jansen was fatally injured on Wednesday, November 11, 2009 around 2:30 p.m. at 2916 Bluff Road, as reported by the Indianapolis Star.

The Indianapolis Metropolitan Police Department reported David Jansen is believed to have either fallen, or was somehow crushed by a moving part attached to a piece of machinery. Jansen was discovered by fellow co-workers laying on the ground. Responding emergency medical services (EMS) teams pronounced Jansen dead at the scene of the accident. Medical professionals stated he appeared to have sustained a potential TBI. The Metro Police stated, “What we think may have happened is he may have become entangled in a piece of equipment which caused his traumatic injury or he might have had a medical event prior to that causing him to fall into the equipment which resulted in that injury.” An autopsy is scheduled to help determine if he had a medical episode before the accident. Jansen reportedly worked for Horner Electric for four years, and has been employed by them in previous years. Federal officials from the Occupational Health and Safety Administration (OSHA) http://www.osha.gov/ are conducting a full investigation into the fatal accident.
