Peoria, IL — A work-related accident at Keystone Steel & Wire Co. killed a man from Cuba on Sunday, July 31, 2011. The worker was pronounced dead following the accident at 3:30 p.m., reported PJStar.com

According to police officials, Keystone maintenance worker, David Hezlet, 51, was killed while driving a golf cart through the mill section of the plant, and veered off course. The golf cart hit a horizontal I-beam, pinning Hezlet in the golf cart.

Responding emergency crews rushed Hezlet to OSF Saint Francis Medical Center emergency room where attending doctors pronounced him dead.

Investigators are looking into what caused the golf cart to veer off path, causing the fatal work-related accident.

An autopsy in scheduled for Monday. Officials with the U.S. Occupational Safety and Health Administration (OSHA) are looking into the deadly work-related accident.

This fatal work-related accident marks the second fatal accident at Keystone. On July 20, John M. DeCanter collapsed and died while in the shower facility. Investigators noted that the intense heat was a factor in DeCanter’s collapse. An autopsy later revealed that a heart attack was the cause of death, and heat was ruled out as a contributing factor in his death.
