The fatal workplace accident took place in Dix, New York, where a maintenance worker had been performing some routine work on a radio tower structure when it suddenly toppled over and fatally injured him.

New York police and OSHA investigators soon arrived at the scene to determine what caused the fatal workplace incident. The Schuyler County Sheriff’s Office reported that the worker had been secured to the radio tower when it collapsed for unknown reasons. The incident occurred in Central New York, 60 miles southwest of Syracuse. The loved ones and family of the worker were notified of victim’s tragic death.
